When requesting to onboard a new volunteer, the requestor/supervisor/PI should complete the form below, which will be forwarded to Volunteer Services. Once received, the required registration documents and onboarding information will be sent to your new volunteer for completion. If you are onboarding multiple volunteers, please submit a form for each one.
- VOLUNTEERS TRANSITIONING FROM REMOTE TO ON SITE WORK: If, at any point, your volunteer transitions from REMOTE to ON SITE work, they would be required to submit all MEDICAL REQUIREMENTS PRIOR to doing any on site work. Simply send an email to Volunteer Services (NYSPIVS@nyspi.columbia.edu) advising of the same. Once all the necessary documents are submitted, a CLEARANCE EMAIL will be sent out advising that your volunteer is cleared to work ON-SITE.
- VOLUNTEER REGISTRATION CLEARANCE EMAIL: Once the individual has submitted all of the required documentation, a CLEARANCE EMAIL will be forwarded to the requestor/supervisor advising of their registration status clearing them to begin working REMOTELY or ON SITE
- Please note: Volunteers aren’t permitted to begin working in any capacity without proper clearance
- ID BADGES: Only once your new volunteer has been CLEARED, Volunteer Services will forward your new volunteer the info on how to obtain their NYSPI ID badge.
- Please note: ID badges are only issued through Volunteer Services. The Safety Dept. will not issue an ID Badge if the request isn’t initiated by Volunteer Services.
- NYSPI EMAIL ACCOUNTS: If the question on the following page regarding an email account is answer YES, psyIT will be included on the Clearance Email which will automatically initiate the setup of the new email account.
- Please note: PsyIT will not process requests for email accounts if it isn’t initiated by Volunteer Services.
- UPDATE TO PREVIOUS REQUEST SUBMISSION: Did any requirements change for your volunteer after last volunteer request submission? If yes click here to notify Volunteer Services with the updates instead of submitting a new request.
